Safe plane for mid-face — depth and landmarks?

For a supraperiosteal bolus at the mid-face, how are you confirming depth and staying off the facial/angular vessels? Cannula vs needle preferences welcome.

For mid-face/cheek, the safest primary plane is supraperiosteal (on bone) with a slow, aspirated bolus, or well-controlled subdermal work — it's the intermediate planes and the pathway of the facial/angular and infraorbital vessels that get people into trouble. Palpate landmarks, know where the infraorbital foramen sits, consider a cannula for the more superficial layers, and inject slowly with low pressure.

  • Cannula changed my mid-face confidence enormously — fewer bruises and I feel much safer around the vessels.
