The three foundation areas — your go-to starting doses (glabella, frontalis, crow's feet)?

For the licensed upper-face areas, what starting doses are people using? I've been taught to start conservative and review, just want a sense-check of typical numbers in the UK.

    Using onabotulinumtoxinA (Botox) unit numbers as the reference, the licensed/commonly-taught starting points are: • Glabella (frown): ~20 units total, classic 5-point pattern (procerus + medial and lateral corrugators). • Lateral canthal lines (crow's feet): ~12 units per side, 3 points, kept lateral to the orbital rim. • Frontalis (forehead): more variable — start conservative, often 10–20 units spread across the forehead, and always assess brow position first. Two rules that matter more than the exact numbers: (1) start low and review at two weeks — you can always add, you can't remove; (2) when you treat frontalis, treat the glabellar complex too, otherwise the depressors are unopposed and you can drop the brow. Always individualise and follow the product SPC.

  • The 'treat glabella whenever you treat frontalis' point is the one I wish I'd internalised sooner. My early brow-heaviness cases were nearly all forehead-only treatments.

  • And keep frontalis injections high enough — staying well above the brow (roughly ≥2cm above the orbital rim) is what keeps you out of ptosis trouble.
