Gummy smile is high-reward but low-margin-for-error, so start conservative. Target the levator labii superioris alaeque nasi (LLSAN) region — the 'Yonsei point' approach places a small dose lateral to the ala where the lip elevators converge. Typically ~2–3 units per side to start. Key cautions: • Assess the type/height of gingival show first — anterior vs posterior gummy smiles behave differently, and toxin suits the anterior/elevator-driven ones best. • Too much or too medial and you weaken lip elevation → a long, flat or asymmetric upper lip and an odd smile. This is very visible, so under-dose and review. • Symmetry matters — dose both sides equally and reassess at two weeks. If in doubt, treat lighter than you think and bring them back.

Photographing the full smile pre-treatment is essential here — small asymmetries you didn't notice become 'you caused that' at review unless they're documented.
